Abstract: 

Kawasaki disease is asystemic vasculitis of unknown etiology.The most significant sequela of acute kawasaki dislease are related to inflamation of small to meduim sized arteries and, in particular, the development of coronary artery dilation and aneurysms (CAA). The aim of this study was to assess the cardiovascular Complications of Kawasaki disease in IRAN. The studied population consisted of 54 patients who were diagnosed to have Kawasaki disease and referred for cardiac evaluation.There were 30 boys and 24 girls, aged between 5 mounth and 13 years (mean 3.9 years). All patients except 3 had received standard treatment with high-dose aspirin and intravenous y globulin (IVGG). All patients were evaluated by physical exam,chest-Xray, ECG and echocardiography (ECHO). abnormal cardiac ,findings included: cardiomegaly in 4 (7.6%) , ECG changes in 4 (7.6%), mitral regurgitation in 3 (5.5%) LV dysfunction in 2 (3.7%) , pericardial effusion in 4 (7.6%) and coronary artery dilatation in 5 (9.2%). Conclusion: 1) Kawasaki disease and its cardiovascular Complications are not uncommon in IRAN. 2) cornonary artery involvement is a major problem. 3) WGG therapy reduces the risk of cardiac Complications. 4) ECHO is a valuable tool in acute phase and follow - up period.

Abstract: 

Introduction: Congenital toxoplasmosis is caused by acute toxoplasma gondii infection of mother during pregnancy, which may lead to such serious Complications as stillbirth, ophthalmologic and neurologic disorders.The aim of this study was to determine the Incidence of toxoplasmosis in pregnant women and their neonates, and disease Complications.Methods: In this prospective cohort study, blood samples of 798 pregnant women were assessed for antitoxoplama IgG and IgM antibodies by ELISA during 2007 -2009. Subjects suffering from acute maternal infection with toxoplasma gondii were considered as case group and those with chronic infection as control group. Nested PCR and ELISA/IgM were performed on four neonates of case group and five samples of control group. Demographic data of mothers and weight and neonates’ gestational age in case (4 subjects) and control groups (28 subjects) were recorded. All infants were followed for clinical symptoms until one year old. Data was entered SPSS and analyzed by Fisher exact test, chi-square and t-test.Results: Five out of 798 pregnant women (0.6%) were positive for IgM 1/100 and suffered from acute toxoplasmosis (case group). Nested PCR showed a 400 bp band in three neonates of case group which confirmed congenital toxoplasmosis, but it was negative in control group (P=0.048). The Incidence rate of congenital toxoplasmosis was 3.7 in 1000 live births. Positive ELISA/IgM wasn’t observed in neonates of case and control groups. Only one infant of case group with congenital toxoplasmosis had icterus (P=0.12).Conclusion: Toxoplasmosis in neonates was diagnosed by nested PCR. Icterus was the only sign in infants with toxoplasmosis. In order to prevent this disease, follow–up and treatment of infants, screening of mothers during pregnancy and their health education before pregnancy are suggested.

Abstract: 

One of the dangerous Complications of cataract extractions is vitreous loss. The purpose of this stuy is to evaluate the Incidence of vitreous loss and its sepuale while teaching residents at Rasoul Akram Hospital in Iran. This study has been conducted on existing records of 730 eyes which were subjectd to extracapsular cataract extractions by 3rd and 4th year, right handed resident of Rasoul Akram hospital. The information was taken from the records of patients who had vitreous loss during cataract surgery and the following data was extracted. The presence of any systemic diseases, phase of operation where vitreous loss occurred, vision on the next day, one month and 3 months later, the cause of low vision and any post operative sequale of the eye were evaluated. Extracted data collected and the prevalence of vitreous loss and its sequale were indentified and presented. Forty-nine eyes (6.7%) had vitreous loss, thirty-one of them were in the left eye (63.3%) and eighteen of them were in the right eye (36.7%). Regarding the stage of operation in which vitrous loss had occurred: twenty –eight eyes while nucleous delivery (57.7%) eighteen eyes during aspiration of cortical material (36.7%) and in three cases during IOL implantation. Five eyes had pseudoexfoliaton syndrome, four patients were diabetic and three had high myopia. Post operational sequale of these 49 eyes were: thirty-five with transient corneal edema (71.4%). Nine with misshaped pupils (18.9%), four with high astigmatism (8.1%), tow with cystoid macular edema (4%), two with retinal detachment (4%) and seven with cloudy vitreous. (14.3%). The Incidence of vitreous loss is more than expected. More extensive research to evaluated the prognositc factors according to the eye (OD/OS) and the stage of the operation the leads to vitreous loss and to decrease its Incidence is still needed.

Abstract: 

Background: About one-third of patients with cirrhosis have gallstones. Gallstones in these patients are often asymptomatic and discovered incidentally, and only 1– 2% per year will develop Complications. We aimed to assess the frequency of gallstone Complications in Egyptian patients with cirrhosis and compare them to gallstone Complications in those without cirrhosis. Materials and Methods: Our study included 120 patients with first-time discovered gallstones. They were selected from patients admitted to the outpatient clinic or the Tropical Medicine Department of Zagazig University, Egypt. They were classified into two groups. The first group comprised 60 patients with cirrhosis with gallstones and the second group comprised 60 healthy individuals with gallstones. Risk factors of gallstones were assessed. Upon admission, all patients underwent a thorough history and clinical examination. Moreover, liver function tests and pelviabdominal ultrasound were done and triglyceride and amylase levels were measured to confirm the presence of gallstones. They were followed up for five years to assess the frequency of gallstone Complications. Results: The group with cirrhosis had significantly lower gallstone Complications than the non-cirrhotic group (8% vs. 41%, p < 0. 001) as regard the overall Complications after 5 years of follow-up. We found that there was a significant difference regarding each complication separately. Acute cholecystitis was (5% vs. 16. 7%, p = 0. 03). Acute pancreatitis was (1. 6% vs. 13. 3%, p = 0. 01). Obstructive jaundice was (1. 6% vs. 11. 7%, p = 0. 02). Conclusion: Our study revealed that the frequency of gallstones Complications in patients with cirrhosis was much lower than patients without cirrhosis with gallstones after 5 years of follow-up.

Abstract: 

Introduction: General anesthesia and surgery cause several physiological disorders in different organs of the respective patients that may appear as several Complications during recovery, which need to be identified and evaluated.Objective: Survey of the Incidence of such Complications at Post Anesthesia Care Unit (PACU) 1 and some of the effective risk factors in Rasht Razi Hospital.Materials and Methods: In this cross-sectional and descriptive study, 155 patients 10-75 years old, in ASA 1 and 2 classes were selected in the hospital. Cases included those who underwent general anesthesia with the same techniques for elective urology surgery and transferred to PACU, in 2010. Questionnaires consisted of two parts, part one included basic information and part two included: respiratory, cardiovascular, neural and digestive Complications. Collected data were analyzed using chi-s quare, paired tests by SPSS 16 and P≤0.05 was considered significant.Results: Findings indicated that the Incidence of considerable changes (increase or decrease) in BP, PR and respiration were 42%, 36% and 49%, hypoxemia (SaO2<90%) 8.4%, pain 76%, shivering 22%, postoperative restlessness 26%, and nausea and vomiting 12% cases, as observed. We found a significant correlation between age and the Incidence of considerable changes of PR and postoperative shivering and also between sex and the Incidence of pain, shivering, postoperative restlessness and delay in recovery. In addition, the data showed a significant relationship between duration of surgery and shivering, postoperative pain severity and delay in recovery.Conclusion: The relatively high Incidence of Complications in recovery indicates the importance of employing skilled personnel and also using enough monitoring equipment at PACU in order to decrease mortality and morbidity of the patients and also save on hospital charges.

Abstract: 

Introduction: Carbon dioxide (CO2) laser is an available technological tool for laryngeal surgeons. Given the importance of laser outcomes and Complications, this research was conducted to identify the Complications associated with laser surgery in the patients referring to the 5th Azar Medical-Educational Center in Gorgan, Iran, from 2013 to 2017. Methods: This descriptive cross-sectional study was conducted on 85 patients with laryngeal lesions who underwent microscopic surgery with CO2 laser at the 5th Azar Medical Educational Center in Gorgan, Iran, from 2013 to 2017. The patients were examined for the associated short-term Complications 1, 7, and 21 days after the operation. They were also examined after 3 months to determine the long-term Complications and patient satisfaction with sound quality. The data were analyzed in SPSS software (version 16). A p-value less than 0. 05 was considered statistically signifi cant. Results: Out of the 85 patients undergoing laser surgery, 60 (70. 6%) cases were male. None of the patients experienced short-term Complications. In terms of long-term Complications, 2 (2. 35%) cases, who had failed to perform the follow-up, had recurrent lesions after 3 months of surgery. In addition, 18 (21. 8%) subjects had recurrent hoarseness, 18. 8% (n=16) and 2. 35% (n=2) of whom were detected with laryngeal carcinoma and recurrence, respectively. However, a total of 67 (78. 91%) cases did not suffer from long-term Complications. In this study, out of 85 patients undergoing larynx surgery, 69 (81. 2%) cases had benign pathology, while the rest (n=16, 18. 8%) had malignant pathology. Conclusions: It seems that a CO2 laser surgery that follows scientific principles can produce good outcomes.

Abstract: 

Background: Wound-related Complications are major sources of trouble in post-operative period and slow down the wound healing process. This study was performed to determine the Incidence of early wound related Complications in laparoscopic versus open abdominal surgeries.Methods: Medical records of 104 elective laparoscopic (A) and 106 diagnosis matched open surgeries (B) including appendectomy, cholecystectomy, ventral hernia repair, and bariatric surgery were retrospectively reviewed between Jun 2002 and Jan 2005. Study data included patients’ sex, age, wound class, type of operation, and occurrence of early wound related Complications. Surgical wounds were evaluated for presence of early Complications during the post-op period and 10 to 15 days after the operation.Results: The two groups were not different regarding age, sex and wound classes. Wound infection developed in 7 patients [RR: 0.06 (0.01-0.77) (95%CI)] and none in patients of group B. Incidence of Hematoma was similar in the two groups; one case in each. No patients in the two groups experienced seruma nor wound dehiscence. Gender, age, and wound classes were not associated with higher rates of wound Complications. Conclusion: Laparoscopic surgery significantly reduced the Incidence of early wound Complications, namely wound infection, and is a safe and efficient alternative to conventional open procedures.
